Attention Student-Athletes and Parents:
This information is for all students interested in athletic participation at Hackensack High School for the 2018-2019 school year.
Registration for all sports is now open. The 2 step process described below must be completed in order to be approved for participation.
- Visit www.familyid.com to complete your online sports registration for all sports.
- Visit www.hackensackathletics.com to print, complete, and submit to the Athletic Office one of the appropriate state mandated forms. Printable form can be found under the ‘SPORTS REGISTRATION’ tab. Descriptions of each packet is as follows:
PPE Medical Packet: Pre-participation Physical Exam (PPE) Packet. This packet is for athletes needing a valid physical exam that will be performed by your primary physician. If your primary physician is in New York and does not sign under the required Cardiac Module, your form is not complete and will not be accepted. Please be advised these physical packets are required to be approved by our school physician; please allow up to 2 weeks for this process.
HHQ Medical Packet: Health History Questionnaire (HHQ) Packet. This packet is for athletes who have a valid physical on file at HHS.
“Valid” is defined as a medical exam that has been conducted within 364 days prior to the official season start date for the sport they wish to participate (official season start dates are posted on high school website)
Physicals provided by our school Physician require the SPP Medical Packet AND parental consent on the Family ID registration BEFORE the date of the physical. Sports physicals will be held in the field house on the following dates:
Wed, May 30th - by appointment only
Wed, June 20th @ 1:00pm
Wed, July 11th @ 10:00am
Wed, July 25th @ 10:00am
(Incoming Freshmen only permitted to July physical dates)
IF THE REQUIRED SPP PACKET AND FAMILY ID REGISTRATION IS NOT COMPLETE, YOU WILL NOT BE PERMITTED TO HAVE A PHYSICAL ON THESE DATES.
*** DEADLINE FOR FALL SPORTS REGISTRATION IS JULY 31st ***
We strongly encourage you to have a sports physical done between the months of May - July to have a valid physical on file for the entire 2018-19 school year, including athletes that only participate in winter and spring sports.
Please be advised of the state guidelines for academic eligibility. Student athletes must pass 30 credits at the end of the school year. This will allow participation in the fall and winter seasons of the following school year. Student athletes must pass 15 credits at the end of the 1st semester to be eligible for the spring season.
